Human Oversight

Human oversight is a checkpoint where a person can meaningfully approve, reject, or intervene.

It only counts if the reviewer sees the evidence behind a proposed action, rather than only the proposal.

Rejecting must be genuinely as easy as approving, or the checkpoint is oversight in name only.

A near-100% approval rate usually means the design failed this test, not that the system is reliably right.
